Fit notes (previously known as sick notes)

To request a fit note please contact us via email at admin.unityhealth@nhs.net, please include the condition that you are requesting the fit note for and the date you need it to begin. Please note that we can backdate fit notes but we cannot forward date.

If you are requesting a fit note extension, we may send you a request for further information to help the clinician deal with your request more efficiently. 

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
